2016 Golden Carrot Award Nomination Form
The Golden Carrot Award by Go For Health! recognizes restaurants that go the extra mile to assist Santa Cruz County residents in making healthful food choices when they eat out. The award is given to restaurants that meet high standards in food safety and nutrition.
Nomination forms are due by December 4, 2015 to the Santa Cruz County Health Services Agency (contact information below).
