1. I have read and understood the information about this survey in the online course.
2. The nature of the survey and its possible side effects has been explained to me.
3. I understand that the survey involves a questionnaire of about 5 minutes, where I will be asked to share my personal views, experiences and attitudes relating to property management planning.
4. I understand that my answers / comments will be recorded electronically (text).
5. I understand that the information that I share in the survey will be treated as confidential and that this does not put me at any risk beyond the everyday level. I understand that my anonymity will be protected in any publication resulting from the survey.
6. I agree that the data collected during this questionnaire may be published provided that my identity is protected.
7. I understand that all data (both electronic and written) will be securely stored at the Cradle Coast Authority.
8. All questions that I have raised have been answered to my satisfaction.
I agree to participate in the survey and understand that I can withdraw my consent at any time without any adverse effect and can request that any information that I have shared be deleted or returned to me.
