At Priory Medical Group we are always looking for ways to improve our service to you. We would like to know what is important to you when using your GP/Nurse and whether we live up to your expectation. This survey is anonymous so please be as honest as you can.

We have modified our survey to include the new "Friends and Family Test" as our first question. The primary aims of this test are to:

* gather useful feedback from people who use our services that can be fed directly back to Surgery staff in a simple format and in near real time
