Welcome and Instructions
Dear Esteemed Colleagues:
Thank you for expressing an interest in receiving training and technical assistance help under the Prosecutor-Led Diversion Initiative. We have developed this electronic application in the hopes of making this process faster and easier for you. Please complete the following application to be considered for this assistance. We look forward to reviewing your application.
Many of the questions in this application are of the "check the box' variety. Some, however, call for narrative responses. The easiest way to respond to these narrative questions is to separately prepare the response in a Word document and cut and paste the response into your application.
You need not complete this entire application in one session. Feel free to complete it in several sessions if you like. You can end any given session by simply closing the surveymonkey.com website after hitting save on the last question answered. Your existing answers to that point will be saved. To continue it again, simply hit the same survey link that was provided in our original email to you again and it will return you to the page that you were last at. You can navigate backward and forward within your application by using the navigation arrows at the bottom of each page. You may have to scroll to the bottom of the page to see these navigation arrows. You are free to alter any of your previous answers until such time as you hit "Next" on Question 29 (The ready to summit question).
Please remember to also follow the instructions at the end of the application regarding submitting a Letter of Support from the prosecutor from your jurisdiction by email. That email also serves to notify us that you have completed an application via the application portal.
The questions are broken into separate sections which ask you about the demographics of your jurisdiction and the nature of your office, your office contact information, your experience with diversion programs, some details about the program that you want to develop, and your readiness to engage such assistance. We anticipate that the check the box portion of the application will take less than fifteen minutes to complete.
