School Reservation Form - Museum Field Trip Survey

Use this reservation form to request a Field Trip with Job Carr Cabin Museum.
We offer in-person and virtual field trip programs designed for 3rd and 4th grade students.
Museum Field Trips can be adapted for other grade levels.
Our educational programs are Fee-Free thanks to the generous support of donors and sponsors! Donations are appreciated but not required. If your school or PTA would like to make a donation, please contact our staff.

8.Preferred Field Trip format

In-person Field Trip preferred.
In-person visits are typically available Mondays and Tuesdays.
Program is 2-2.5 hours. Maximum 90 students per day.
Tacoma Public Schools may be eligible for bus funding through Tacoma Creates.

Virtual Field Trip preferred.
Virtual programs are typically available Mondays - Thursdays.
Program is 1-2 hours. Multiple classrooms from the same school can participate simultaneously through a secure video link (Zoom, Teams, or GoogleMeet). Classroom microphone required to enable students to ask and answer questions.
