2017 NAMI Maryland Annual Meeting Proxy Form

The NAMI Maryland Annual Meeting will be held on June 11, 2017. Attendees will have the opportunity to elect the NAMI Maryland Board of Directors as well as vote on proposed changes to the NAMI Maryland bylaws. If you are unable to attend the meeting, you may fill out the below proxy form. You must be a NAMI member in Maryland through June 11, 2017 to have your vote counted. Please visit www.namimd.org to renew or become a member.

To designate someone to vote on your behalf, you must complete a proxy form.* You may submit your proxy using this online form. All contact information must be filled out for the proxy to be counted.

The ballot of candidates and proposed changed to the bylaws can be found HERE.

*If you choose not to use this online form, use THIS LINK to obtain a printable version of the proxy form. If you are submitting the proxy form to NAMI Maryland, the form must be received by the NAMI Maryland office by 5:00 pm on Saturday, June 10. Or you can also give the form to the designated NAMI member proxy holder, and they may bring it with them to the Annual Meeting.
