We want to find out how medication shortages are affecting people with epilepsy and Parkinson's.

If you have epilepsy or Parkinson's, or if you are a carer or guardian for someone with one of these conditions, please respond to these questions and help us to gather data on the impact of medication shortages. We’ll use this information to urge the government to take action.

2.Have you/the person you care for had any difficulty getting your medication in the last......?
3.Has the pharmacist been supportive in trying to access medication when there is a shortage?
4.Have you or the person you care for had to visit multiple pharmacies in order to find the right medication?
