Putnam Valley CTC Parent Survey

As part of our community assessment to address those issues that teens face in Putnam Valley, the Putnam Valley Communities That Care Coalition is asking for your help. If you live in the Putnam Valley Central School District, could you please fill out this survey.  This survey only relates to children in middle or high school. Everyone who responds by March 1st will be automatically entered into a drawing to win one of two $50 gift certificates! If you are interested in being entered in the drawing, please type your email at the space provided at the end of the survey and you will be contacted if you are a winner.
