Community Health Access & Systems Study

Participant Information (Please read before beginning)
I am conducting a nationwide research study to understand how people access health information and preventive services—and how everyday conditions (like transportation, food, internet, housing, safety, and work) shape those experiences. Your input will help produce clear, data-driven findings I can share back for learning and system improvement.

What you’ll do: You will complete a one-time survey about your experiences and views. The survey takes approximately 15–20 minutes. You may skip any question after section 1 or stop at any time.

Who should participate: Individuals 15 years or older living in the United States are eligible. If you are 15–17 years old, please participate only with parent or guardian permission.

Privacy & data use: Participation is voluntary. Results will be de-identified and reported in aggregate; individual responses will not be shared. We do not require your name. At the end of the survey, you may optionally provide your name and email if you wish to receive findings or future updates. We ask for your ZIP code or your city and state to understand regional patterns. All data are stored securely.

Possible sensitivities: Some items touch on personal or community topics (e.g., safety, substance use, sexual/reproductive health). Again, you can skip any question after the section on demographics.
