Thank you in advance for taking the time to complete this national quality improvement survey to provide valuable feedback to plan upcoming activities (Jan-Jun 2020) by the Center for Quality Improvement and Innovation (CQII). We estimate that it will take 5-10 minutes of your time to complete this survey.

For the purpose of this survey, we are using the following definitions:

Quality improvement – specific activities that are aimed at improving the quality of care and staff/patient experiences at your organization.
• Quality improvement capacity - Beginner-low understanding of quality improvement/limited quality improvement activities; Intermediate-good understanding of quality improvement/experience with quality improvement projects; Expert-well versed in quality improvement methodologies/ability to teach quality improvement to others/lead implementation of quality improvement projects.

Your responses are greatly appreciated to help CQII to effectively meet your quality improvement needs. If you have any questions, please contact us at Info@CQII.org.
