FEES AND TERMS OF AGREEMENT
We understand that the individual price is $1,500 for a 6’ Table Top Exhibit, 2 chairs, and a wastebasket. Tables will be assigned on a first come, first serve basis. A final floor plan with table assignments will be forwarded prior to the meeting. Any additional services such as electrical, Internet, and shipping are at the exhibitor’s own expense. Applications must be received by February 17, 2023 to be included in Exhibit Guide.

PAYMENT
Please make checks payable to the Association of Program Directors in Vascular Surgery (APDVS) and mail to: APDVS, 35313 Eagle Way, Chicago, IL 60678.

REFUNDS/CANCELLATION POLICY
Cancellations must be received in writing by February 24, 2023. All refunds in exhibit fees are subject to a 25% administrative fee. There will be no refunds for cancellations received after February 24, 2023.
