The Planning, Institutional Effectiveness, and Research Department (PIR) strives to provide our customers with accurate and clearly presented information in a timely fashion. PIR provides this form for handling and tracking all survey requests. Surveys should be requested at least three weeks prior to the requested start date to accommodate implementation. For this reason, it is very important that requests are made as far in advance as possible. Please give us as much notice as possible when requesting surveys.

Prior to submitting your request, please review the information already available on PIR’s Intranet site. You can view the readily available information by clicking on the items on the navigation bar on the right side of the page. 

If you are unable to locate the information you are looking for on PIR’s Intranet site or need additional information, please complete the Survey Request Form, please be as specific as possible to ensure our accurate understanding of your request. 

Additional Notes
  • If you have submitted this type of request before, please send to the (in addition to completing this survey request form) a copy of the report or data file that we provided to you so that we may use it as an example to help us fulfill your request. 
  • Please note that some research projects need to be reviewed by CSM’s Institutional Review Board (IRB) and submitting for that review is a separate process from completing and submitting this Survey Request Form to PIR.  Please visit for additional information about the IRB review process.
  • When the PIR Department is fulfilling an external survey request, no contact information for students under the age of 18 will be provided. 
  • PIR makes every attempt to make surveys as accessible as possible.
  • All final survey reports will be posted on the PIR intranet page unless otherwise noted.
