Good health is created in the places where we live, learn, work, and age.

The North Carolina Perinatal Health Strategic Plan (PHSP) seeks to improve the health of all North Carolinians, reduce infant mortality, and eliminate racial disparities in birth outcomes through collaboration with partners to promote the social and economic conditions that support good health.

We would like to learn more about work that is happening statewide and in local communities to address the factors that contribute to North Carolina's biggest health challenges. The following survey collects information about your organization's activities related to the Strategic Plan's three population-level goals:

1. Improve health care for women and men
2. Strengthen families and communities
3. Addressing social and economic inequities

Please take some time to share your organization's work in these areas. Your information will be used to help better understand current statewide and community-level health promotion efforts and identify gaps and potential partnerships.
