Hello,

My name is Sarah Herbert and I am a Family Nurse Practitioner student at University of Nevada, Reno. I am working on my masters project by conducting a research study to learn about the process of assessment for the Patient Health Questionnaire 9 and modified 9A (PHQ-9 or PHQ-9A) depression screening tool for adolescents ages 12-17 years old in the state of Nevada.

If you volunteer to be in this study, you will be asked to answer 16 questions regarding your demographics, specialty and what type of interaction you have with adolescents as well as if you yourself utilize the PHQ-9 or PHQ-9A depression screening tool or if you utilize a di2erent type of depression screening and why.

Your participation should take about 3-5 minutes to complete.

This study is considered to be minimal risk of harm. This means the risks of your participation in the research is similar in type or intensity to what you encounter during your daily activities.

Benefits of doing research are not definite; but we hope to learn how providers in the state of Nevada are assessing adolescents 12-17 years old for depression and to see if there is a consistent manner in which it is being accomplished or if there is a lack in assessment. There are no direct benefits to you in this study activity.

The researchers and the University of Nevada, Reno will treat your identity and the information collected about you with professional standards of confidentiality and protect it to the extent allowed by law. You will not be personally identified in any reports or publications that may result from this study. The US Department of Health and Human Services, the University of Nevada, Reno Research Integrity Office, and the Institutional Review Board may look at your study records.
