The Port Gamble S’Klallam Tribe is administering a community survey to gather information about the needs and opinions of adult tribal members and other adults who live on the reservation about various issues. This information will be used to create a picture of the community as a whole, to guide and plan for services. We do not keep track of your name, address, or personal information that identifies you or your family. All respondents will be confidential, and all responses will be pooled together with the responses from other respondents.
Your participation in the survey is voluntary. You don’t have to answer any question you don’t want to, and you can stop the survey at any point. We expect the survey should take about 15-20 minutes and for your time we will provide you with a $25 cash incentive. At the end of survey, you will have the opportunity to provide additional information to receive the incentive. All information you give us will be confidential.
