The Paths to Employment Resource Center (PERC) Research Team would like to gather information about your role or function in helping a person with a disability to reach integrated employment goals.
The purpose of this survey is 3-fold: 1.) To gain a clearer understanding of your current role and responsibilities. 2.) To identify the frequency, importance, and competency of certain activities you perform. 3.) To determine potential training or resources that PERC may provide to help you in daily activities.
This survey is part of PERC's ongoing research project that focuses on employment services for people with disabilities. It has been approved by the School of Education (SoE) Institutional Review Board (IRB) as required by the Code of Federal regulations Title 45 Part 46. By responding to this survey, you are consenting to participate in this research study as a participant.
This survey is entirely VOLUNTARY and all your responses are ANONYMOUS and KEPT CONFIDENTIAL! We hope you will take some time to answer the following questions. If you do not want to answer any of the question(s) in this survey, you can choose to skip the question(s).
Reminder: It is possible that you MAY or MAY NOT feel competent in some of the areas. It is possible that you MAY or MAY NOT have to do some of the tasks at your workplace. It is possible that you MAY or MAY NOT want to answer some of the questions.
We estimate that it will take approximately 15-20 minutes to complete this survey. We strongly encourage you to complete the survey in one session. However, if necessary, you can complete your survey in several sessions as long as you are using the same computer.
