Secondary Transition

In Pennsylvania, secondary transition starts at 14 years old. If your teen/young adult uses special education services Individualized Education Program (IEP) or has a disability (504 plan) it’s important to start early in planning to support their vision of transition to adulthood.

There are multiple state systems and community resources that may be able support their goals by providing services to help make the secondary transition to further education, employment and community life even before they graduate. The planning and process should be based on the strengths, dreams and goals of your teen/young adult. There is a lot to learn about what is available in working with educational, vocational and community partners to empower young people in transition to adulthood.

Parents have a lot of questions about the changes that are about to occur. This survey aims to understand what PEAL’s 21 and Able Project can do to support families and caregivers in the secondary transition process.
