Thank you for your ongoing participation in AEDP Research. The following scales should take you approximately 5 to 10 minutes to complete. These scales should be completed as soon as possible following each of your therapy sessions.

Please input your user ID. Your user ID should be your patient’s first and last initials, your last name and the last four digits of your social security number.

For example: if your patient is Anna Smith and your name is Jenny Young, and the last four digits of your social security number are 9999, your user ID would be ASYoung9999.
