Personal Demographics Service Application Form

In order to activate the Personal Demographics Service (PDS) for your project, we will need to confirm you have met the requirements mandated by NHS England Digital. Please complete and submit the following form, we can then proceed with setting up PDS for you.
1.What is your email address?(Required.)
2.What organisation are you part of?(Required.)
3.Has your organisation read and understood the End User Organisation Acceptable Use Policy [EUOAUP] for the Personal Demographics Service? (https://digital.nhs.uk/binaries/content/assets/website-assets/services/operations/changes-summary/connection-agreement-appendix-1a-euoaup.pdf ) This is a necessary requirement before being given access.(Required.)
4.Has your organisation completed the DSP Toolkit? We must verify this before you can be given access to the Personal Demographics Service.(Required.)
