PDGM Comments, Issues and Experiences

Now that the Patient Driven Groupings Model (PDGM) is several weeks into implementation, it's vital for HCA to learn about and track any issues that providers may be experiencing in the field, whether positive or negative.

HCA is working with the National Association for Home Care and Hospice to collect PDGM implementation experiences and work to resolve any issues, if necessary, with CMS and contractors. Please tell us anything we need to know or what we can do to help.
