Phoenix Day 2021 Block Party Eligibility Form

Thank you for your interest in hosting a block party with your neighbors for Sunday Streets Phoenix Day on October 17, 2021 from 12:00pm - 5:00pm!

To complete This Form, you will need to:
- Upload a letter of support with signatures from 60% of the households on the block.
- Identify the district in which your block is located. If you don't know which district you live in, visit: https://sfplanninggis.org/SFFind/?
- Certify that you have read the program guidelines and agree to adhere to all of them.
