Adams County Community Health Assessment Survey 2025

Adams County- Public Health is currently conducting a Community Health Assessment, and would like to hear directly from you, our residents, workers, and community members.

Public Health is interested in your perspective on what’s going well in Adams County, as well as the challenges or areas that need improvement when it comes to health and well-being. Your insights will directly influence our priorities and guide our public health efforts for the next five years.

The information collected from this survey will be used to:
· Identify key health issues in our community
· Develop meaningful, community driven solutions
· Support the creation of new programs, policies, and services
· Guide collaboration with local and state leaders
· Empower local action teams to implement real change

The survey will take you roughly 5 minutes and after completing the entire survey, you’ll be entered into a drawing to win one of six $25 Kwik Trip gift cards.

Deadline to participate: August 28th, with all responses being confidential and remain anonymous.
