Virginia Block Grant Needs Assessment Process Survey |
This survey is intended to gather feedback from CSBs about the community needs assessment process that you recently completed as part of the Virginia Block Grant. The feedback that you provide will help us better understand successes and challenges of the process, and allow us to improve technical assistance and support around future Block Grant (BG) efforts.
You are being asked to participate in this survey as the primary point of contact for your BG CSB. You will not be asked to provide any identifying information, and your responses will not be linked to your specific community - so please answer honestly!
