Every trainee is given a unique identifier which will not be linked to your name or any other identifying information. The reason we ask for a unique identifier is so that we can anonymously link your survey responses to survey data for this project which we use for the purposes of understanding how valuable physicians find these offerings and whether they engage in office based treatment of opioid dependence and whether this changes with time.

The unique identifier is a series of 4 letters: 1. first initial of your mother's first name, 2. first intial of your mother's maiden name, 3. first letter of the town/city in which you were born, 4. first letter of the state/country you were born in.

Example: Mother's Name: Jane Doe, Your Birthplace: Anywhere, California
