Welcome to the 2021 Clinic Practices in Depression Care Survey. The survey is part of a project that is being funded by a prestigious Eugene B. Washington Community Engagement Award from the Patient-Centered Outcomes Research Institute (PCORI), with leadership from MN Community Measurement. This survey is sent to all medical group primary contacts registered with MN Community Measurement. The survey should be completed by each unique clinic site as registered in the MN Community Measurement data portal.

The survey is open between February 17, 2021 and March 17, 2021.

Survey results will be used to:
  • Identify clinic practices that contribute to higher rates of performance on the depression suite of measures publicly reported by MNCM;
  • Guide development and communication of programs and resources locally, statewide, and nationally to help patients with depression;
  • Engage communities and stakeholders in collaborative efforts to improve mental health care and outcomes statewide; and
  • Support quality improvement initiatives at your clinic
This is a voluntary survey. However, given the rising prevalence of mental health conditions during the pandemic and the continued challenges in helping patients with depression achieve remission, we hope that you will take 10-15 minutes to complete this survey to share your practices and insights.

Your participation is entirely confidential and will increase the value of the findings on this important topic. No calculations or the need to look up clinic data is necessary--please use your best judgment in responding to the questions based on your knowledge of your clinic and its operations.
