Practical Church Leadership Program
Scholarship Application 2025

Please review the United Methodist Foundation of Louisiana's Practical Church Leadership brochure and the program information on the Dakota Wesleyan University website to familiarize yourself with the requirements before completing this application. Applications must be submitted by April 15 to be considered for the cohort beginning in July 2025.

3.Please list your post-high school education (degrees received, year, institution). List all degrees received. If you have no post-high school education,  provide your high school name and city, and the highest grade completed. (NOTE: If you are selected to receive this scholarship, you will be required to submit official transcripts to DWU for admission to the program.)

The Practical Church Leadership Certificate Program will require a significant time commitment.

8.I understand that this program includes 21 hours of online graduate-level college credit, and I am ready to commit the time it will take to complete the program requirements.
9.The Practical Church Leadership Certificate Program requires travel for three (3) in-person gatherings during the year-long program: July, January and July. Are you prepared to clear your calendar for these two-day, expense-paid gatherings and make this a priority? (Note: the location may require two (2) additional travel days.)

I also understand that if I do not complete the program, the United Methodist Foundation of Louisiana may submit an IRS Form 1099-MISC to me for the expended costs associated with the program.
