PCDC Community Survey 2016: Disclosure Statement

PCDC is a 501(c)3 nonprofit. We have created the PCDC Community Survey, an easy and important way you can help your community. It is a quinquennial opportunity to voice your opinions on what you like about your neighborhood and what changes you'd like to see. This will shape the services we offer, including affordable housing, education, events, healthcare, and sanitation. It also provides valuable demographic data that will be made available to the public. Some data may also go to the City in support of the Affirmatively Furthering Fair Housing law.
You do not have to answer all the questions and can end the survey at any time. However, all who complete a survey have a chance of winning one of four $50 Visa gift cards.

All individual survey data will be kept confidential. Only aggregate data will be released to the public. You must be at least 18 years old and a community resident to participate. This survey is only available to randomly selected residents, so we hope you take this special opportunity to speak up for Chinatown.

You have been randomly selected to participate in this survey because you are a resident within Chinatown’s borders (Spring Garden, 8th, Filbert, and 13th) and Callowhill. You will be asked to complete a 15-minute survey about yourself and your opinions on community issues.

Survey Purpose:
PCDC is conducting the Neighborhood Survey to learn more about how residents of our neighborhood feel about a number of community issues. The information from this survey will inform the community Neighborhood Plan 2016, which will guide PCDC programs and activities in the upcoming years. Additionally, some questions from this survey will assist the City of Philadelphia’s Assessment of Fair Housing report, in compliance with HUD’s Affirmatively Furthering Fair Housing law.

PCDC, founded in 1966, has a long history of protecting, promoting, and preserving Chinatown as a viable ethnic, residential, and business community. PCDC is responsible for projects such as the Chinatown Friendship Gate, 10th Street Plaza, the On Lok House affordable senior home and center, and 319 mixed-income housing units. PCDC organizes the annual Yè Shì night market and Chinese New Year events which draw close to 30,000 visitors annually. Our $1-a-Day cleaning team of two persons work seven days a week to keep Chinatown streets clean and maintained. We also provide free bilingual family support service counseling, housing counseling, and youth programs, among other services. Risks/ Rewards:
The survey is completely voluntary. You do not have to answer any questions you do not want to. If you choose to be in the study you can withdraw at any time without any consequences. There are no anticipated risks to you if you participate in this survey, beyond those encountered in everyday life. Residents who complete the survey will be entered into a raffle for one of four $50 Visa gift cards. Raffle winners will be notified by mail. 

Privacy Statement:
The information that residents provide will be confidential. Personally identifiable information is collected for survey participant rewards and internal data quality control only. PCDC will not share individual responses with anyone, for any purpose, in any way that identifies the individual. 

o   We will not share individual answers or names with any third party or affiliated organization.
o   Only the researcher in charge and research staff will have access to individual survey responses.
o   Individual responses will be combined with others. Only this aggregate data will be shared. PCDC will make the aggregate summary data from the Neighborhood Survey available to participants and the general community on our website www.chinatown-pcdc.org.
