Instructions for taking survey

This survey measures the organizational health of your church. It address such areas as how strategic you are, whether your structure is designed to help you accomplish your mission, and if you are developing leaders and resources to support the ministry of the church. The survey has been designed to a large degree by the findings of the Fishbone Diagram Church Health Assessment tool.
 
Answer each question to the best of your knowledge. If you do not have enough information to answer, or you cannot agree or disagree with the statement, select the answer Undecided. However, if you lean even the slightest one way or the other, please select Agree or Disagree.

* 1. Our church has conducted a disciplined form of strategic planning in the last five years. (Strategic planning is defined as church leaders agreeing on who we are, where we are going, and how we will get there).

* 2. Our current organizational structure was designed to help us accomplish our mission. (Your structure includes your Constitution, By laws, the number of boards you have, and the relationship of boards, Council, staff, congregational members, and your ministries).
