What is purpose of this survey?
Your responses will help us to accomplish the following: Better understand the great work already going on with respect to health in Hampshire County
Better understand who is already collaborating and what new collaborations are needed
Identify areas of need and opportunities for improvement
Identify expertise and resources
Who are we?
We are a steering committee representing a range of organizations and initiatives focused on health improvement in Hampshire County. We have come together to create a County-wide Community Health Improvement Plan that will guide our collective work for the next few years.
What is a Community Health Improvement Plan (CHIP)?
A health improvement plan is a long-term, systematic effort to address public health problems based on community health and other assessments. This plan will be used by health and other public and private human service agencies and community partners to articulate shared priorities and coordinate and target resources that promote health. In this process, we are defining “health” as more than just medical or health care. We recognize that health is determined by a variety of factors, including our environment, access to resources such as education and income, social support and safety where we live, work and play.
Our steering committee has engaged the Pioneer Valley Planning Commission to facilitate this process. Our hospitals have completed a comprehensive health needs assessments and we will be building on those findings for our process.

Please join us at our County-wide convening on October 6, 2017 from 9:00am-12:00pm at the Hadley Farms Meeting House. We will share the findings of this survey, hear from notable speakers, and work together to create the Hampshire County Community Health Improvement Plan.
