This 10-minute survey is being conducted by Proof Alliance NC. The purpose of this online survey is to examine views about alcohol use during pregnancy and to assess awareness of and reactions to the We Have the PROOF statewide media campaign related to this topic. Your input will help inform and improve Proof Alliance NC's work related to fetal alcohol spectrum disorders.
North Carolina adult residents ages 18 and older are eligible to participate. The survey is completely voluntary. You may choose not to participate or you may skip any question you do not want to answer. The information from the survey will be reported as a summary of all responses received, not of individual participants' responses. Please note that fully answering the survey is helpful to our program by providing us with needed data and feedback that will assist us in our outreach efforts.
Survey participants can enter a drawing for chances to win one $25 gift card given out weekly and one $100 gift card given out at the end of the survey period to Walmart, Best Buy, Bath and Body Works, McAllister's Deli, Jason's Deli, or Dunkin Donuts. You must be willing to provide us with up to date contact information to be entered to win.
