International Travel Notification

Families and staff members who traveled outside of the United States are asked to complete the following information for each student or staff member who participated in the trip. Students and staff who are traveling should be aware that District 200 will continue to monitor recommendations and information from the CDC and will follow their recommendations, which could potentially result in self quarantine.   We appreciate your cooperation in helping to keep all of the students and staff of the district safe.

Las familias y empleados del distrito que vayan a viajar fuera de los Estados Unidos deberán completar el siguiente formulario para cada estudiante o miembro del personal docente que vaya a participar en el viaje. Los estudiantes o miembros del personal que van a viajar deberán tener en cuenta que el D200 va a continuar monitoreando la información del CDC y seguirá sus recomendaciones, que podrían resultar potencialmente en una cuarentena voluntaria. Apreciamos su colaboración para ayudarnos a mantener a los estudiantes y personal del distrito a salvo.
