1. Welcome to our survey

The Parkinson Association of Alberta (PAA) is conducting a strategic planning process to determine our future priorities and the best options for meeting the needs of our clients. We are asking for your input to help guide our Board and Strategic Planning Committee as PAA continues to move forward to support people with Parkinson Disease and their care givers. 

In addition to this electronic survey, we are conducting a number of ‘Community Conversations’ from late January to mid-February 2018.  These forums will be held in Calgary, Edmonton, Grande Prairie, Lethbridge, Lloydminster, Medicine Hat and Red Deer. You are invited to offer your input and opinions to both ‘Community Conversations’ as well as this survey.  The schedule for ‘Community Conversations’ is available on the PAA website at www.parkinsonalberta.ca.

All responses will be collected anonymously by a third party consultant at impact8 Inc. and will be kept confidential as per the Information and Privacy statement, below.

This survey was designed to collect information on the perceptions and opinions about the programs and services of the Parkinson Association of Alberta (PAA). The information collected in this survey is in accordance with the Personal Information Protection Act (PIPA) and will be used solely to inform the PAA planning process.

 No identifying information will be requested and a minimal amount of demographic data will be collected. Your participation is voluntary. You have the right to discontinue the survey at any time by exiting the survey or closing your browser.
