Tell us what you want to see in your community

Throughout 2019 the State Disability Plan is being developed through feedback from the community of Western Australia.

Between February and April 2019 we ran face-to-face workshops across the state to define the values of the State Disability Plan and the attitudes and behaviours that will support better inclusion for people with disability across Western Australia. Information received from these workshops will help develop the Plan and also inform a social change campaign to support the State Disability Plan.

If you didn’t make it to a workshop and would like to have your say, complete the following survey to help us shape the Plan.

If you require assistance to complete this form please telephone 6167 8899 and our team will happily assist you.

*Survey closes Tuesday 30 April 2019

Attitudes & Behaviours
An attitude can be defined as a way of thinking or feeling about someone or something, usually reflected in a person's behaviour.

Behaviours are the way in which a person acts or responds, especially towards others.

We have all witnessed or experienced attitudes and behaviours - both positive and negative - that impact on people with disability.

For the positive behaviours, we would like you to tell us how we can reinforce these across the community. For the negative, we would like your input on how we can change them for the good.

Question Title

* What positive attitudes and behaviours have you seen or experienced around disability and inclusion in your community?

Question Title
