* 1. This is the Registration Form for the 2017 FACCS Junior Fine Arts Regional Festival. By completing and submitting this form the school agrees to permit students to participate in the Regional Festival. The school also agrees to permit participants who qualify for the State level competition to attend and represent their region at the State event.

This form must be completed and submitted no later than Friday, April 7th to be considered On-Time. Registrations received between April 8th and April 14th are subject to a late fee of $5 per individual entry, and/or $30 per group entry.

Please do not miss any student or group when you register as NO REGISTRATIONS WILL BE ACCEPTED AFTER APRIL 14, 2017. All payments must be postmarked to the FACCS Office of Program Services or paid online no later than April 21st or late fees will apply regardless of your registration date.

Do not begin filling in this form until you are ready to complete ALL sections (Art, Music Solo/Duet, Music Groups, Speech Individual/Duets, Speech Groups, Multiple Entries).
