Caregiver Peer Mentor Application

Thank you for your interest in providing support to other caregivers by being a volunteer peer mentor through the Lymphoma Canada peer mentoring program. Having been through the experience of caring for a cancer patient, you know firsthand the challenges faced as you learned more about lymphoma, what the patient goes through, and how best to support them and yourself throughout diagnosis, treatment, and follow-up care.

The Peer Mentoring Program is a service provided to Canadian patients and caregivers age 18 and over. The program matches screened and trained caregiver volunteers who have experience with lymphoma (mentors) with caregivers who are seeking support (mentees). This program has been developed to provide a short-term support opportunity for caregivers. In order to provide the best level of support, we match caregivers based on important factors such as location, age, diagnosis, etc.

We have therefore developed this survey application to better understand your lymphoma or CLL experience to best match you with a caregiver looking for support. The information provided in this application is strictly confidential and will only be used to create a peer match. Information provided will not be shared unless permission has been granted by you.
