You can review our most recent 2018 Title V Block Grant Application to review our 5 year action plan in more detail as well as the Executive Summary.  
Your comments and feedback throughout the year are welcome and appreciated! 

The five year action plan is divided into 5 MCH populations (women, infant, children, adolescent, children and youth with special health care needs).  The Life Course domain from previous years was absorbed into the other MCH populations.
