The Adoption Council of Canada and the North American Council on Adoptable Children's Community Champions Network Coalition is conducting this survey to learn how adopted children are doing in different aspects of their lives and what services their families might need. The information will help the Canadian adoption community in advocating for the identified supports and services.
We hope you will take the time to fill out this survey. It is important to hear from as many families as possible, both those who are experiencing challenges as well as those who are not. If you have any questions, please contact Kim Stevens at kimstevens@nacac.org or 508-254-2200.
PLEASE NOTE! IF YOU HAVE ALREADY PROVIDED ANSWERS TO A PROVINCIAL SURVEY (ONTARIO, NEW BRUNSWICK, OR NOVA SCOTIA) DO NOT RESPOND AGAIN. THIS WILL CONTAMINATE THE RESULT BY OVERSTATING NUMBER OF RESPONDENTS! Thank you.
Instructions: If you have more than one adopted child, please answer special needs and services questions in response to the child with the greatest need.
