2017 CSI Retreat - Sponsor Attendee Form

Thank you for committing to the CSI relationship. We're excited to have your firm attend our retreat!

Get your passport ready!  This survey confirms the number of attendees that will join the CSI team on the cruise, as well as the type of rooms your attendees would like reserved.

A separate survey will be sent to the attendees requesting more information including passport numbers, birth dates, t-shirt sizes, etc.

Event Dates:

Carnival Cruise from Long Beach to Ensenada
Friday, September 29th through Monday, October 2nd

Cancellations or changes can be made with no penalty until July 26th. A 50% cancellation fee will apply from July 26th through August 14th. A 75% cancellation fee will apply from August 15th through August 31st. No refund will be provided after September 1st.

Payment:

Based on your selections within this survey, a separate invoice will be sent representing the attendance costs.  This is separate from your firm's commitment to be a Platinum Sponsor.  Attendance costs are in addition to the sponsorship.

This form addresses the cost of the cruise room reservation only.  This does not include transportation costs to and from Long Beach, California.
