Southern Oregon CHNA Listening Session Signup - 2/10 Session

Asante and Providence Health and Services are conducting a Community Health Needs Assessment (CHNA) for the Southern Oregon region, which will result in a set of prioritized needs and guide the community health improvement process. This process is completed every three years to help Asante and Providence Health and Services learn about what is going well and how to better support communities. Asante and Providence Health and Services has contracted with Rede Group, a public health research firm, to facilitate listening sessions for the CHNA.

The listening session will be approximately 90 minutes long and will take place virtually through Zoom on Thursday, February 10th, from 5:30-7:00 PM. Participants will be mailed a $50 VISA gift card after participating in the listening session. Please complete the questions in this survey to sign up for the listening session. The information provided in this survey will not be shared with anyone outside of the Rede Group.
