1. Disclosure of Risk
1.1 I Understand and accept that i will be exposed to substantial risks while using Western Athletic Club Gym.
1.2 My Exposure to risks whilst using Western Athletic Club includes (but is not limited to) that:
1. I may be physically or mentally injured, maimed or killed, in a way that can result in my partial or total disability, paralysis or death;
2. my property may be lost, stolen, damaged or destroyed.
1.3 There may be other risks not known at this time, and which are not reasonably foreseeable at this this time.
1.4 Some of the risks to which i am exposed by using Western Athletic Club simply cannot be avoided by the exercise of reasonable care by the owner.
1.5 While using Western Athletic Club Gym, i will only engage in activities for which i have skills, preparation and training.
1.6 I understand that Western Athletic Club Gym is an unsupervised facility. The fact that it is an unsupervised facility both
a) increases the risk of an incident occurring: and
b) prejudices the ability for me to receive prompt assistance (including medical assistance) if an incident does occur.
1.7 I have disclosed to the Owner/Trainer/School any pre-existing injuries or illnesses which i have, and have provided the relevant and necessary information about my health, ability and capacity to use the gym.
1.8 I voluntarily assume all the risks to which i am exposed to while using Western Athletic Club Gym.
1.9 I will not use Western Athletic Club Gym while intoxicated or affected by drugs.
1.10 I will follow all instructions, rules or directions issued to me by management, coaches, school staff.

* 6. GYM RULES

1. Only persons who have completed this survey induction may use the gym.
2. This gym is not supervised. Persons use the gym at their own risk.
3. Familiarize yourself with the location of the first aid equipment.
4. Your safety is paramount. Always workout in a safe manner, to yourself and to others. Be courteous to other users.
5. Personals should not undertake exercise routines unless physically able to do so. If necessary obtain your doctors approval.
6. Shoes and appropriate exercise attire are mandatory.
7. If you are unsure as to how to use any of the machines, ensure you obtain adequate instructions before doing so.
8. Do not exercise whilst impaired by alcohol or drugs.
9. No smoking is permitted within the gym.
10. Please wipe down all equipment with an antibacterial wipe after use.
11. Return all weights and other equipment to their storage areas immediately after use.
12. Report any malfunctioning or damaged equipment, or any other unsafe condition, to management or school staff.
13. Security camera will be operating inside the gym.
14. No spikes or football boots are to be worn inside the gym area.
15. Please store bags in the bag racks, not on the floor.
16. Children are only permitted in the gym under adult supervision.
17. Anyone letting unauthorized people access the gym will risk permanent dismissal.
