Thank you for your participation in my fiscal year 2023 budget engagement survey. The goal of this survey is to better understand the priorities of Ward 5 residents as I prepare to send my annual budget letter to the Mayor. The budget survey is one of many tools that will be used to engage residents around the budget. The data collected will only be used internally to help identify #OurWard5 budget priorities. Information collected during this survey will not be shared or given to any third parties. 

All surveys must be completed by 11:59 PM on February 18th, 2022. Physical copies of this survey can be found at Woodridge library located at 1801 Hamlin St NE starting January 21, 2022.

*To complete the online survey, an answer is required for all questions except the "Contact Information" question at the end of the survey, which is optional. Some questions allow the survey participants to make multiple selections.
