Sonoma County Older Adults Social Supports Needs Assessment

A needs assessment to understand older adult (60+) access to social activities. Isolation is adversely impacting older adults and responses to this survey will help the County of Sonoma support and improve the health and quality of life for older adults in the time of the COVID-19 pandemic. This survey is for older adults (60+) and for residential workers, caregivers and faith based and/or non-profit partners.
