Interested Primary Care Provider Screener Survey

Thank you for your interest in participating in the AHRQ-funded grant project to expand access to needed pharmacologic and psychosocial services for persons with opioid use disorder (OUD). This project will support primary care providers (PCPs) (physicians, physician assistants, and nurse practitioners) and their practices in adopting medication-assisted treatment (MAT). If you agree to participate in the project, you will be offered a set of trainings, including the required coursework for acquiring a waiver to prescribe schedules III, IV, and V FDA-approved opioid-based medications for the treatment of OUD. Experienced addiction medicine specialists will provide mentorship on how to integrate MAT into your practice.

Practices of various sizes, and specialties are eligible to participate, and previous experience identifying and treating addictions in the primary care setting is not required.
