In early 2020, the Suicide Prevention Plan for Ohio was created by a diverse group of 33 community-based professionals and released alongside the support of Governor Mike DeWine. The resulting plan sets out actions to be implemented over the next three years.

One of the specific goals is to Integrate suicide-specific care across healthcare, behavioral healthcare, and addiction treatment organizations. Your responses will help us take inventory of how healthcare organizations are incorporating suicide prevention education, advocacy, and promotion in organizations across Ohio.

Directions:  For each of the following questions and statements, please choose the best answer based on the patients that your organization serves and the type of suicide prevention services that your organization provides. Each question is optional. We ask, however, that you please complete all or as many of the questions as possible. We estimate that it will take your organization about one hour to complete the survey. Please consult with any staff members who can provide information. You may start the survey and return to finish it later if needed. Simply use the same computer you began the survey with to finish the survey.
If you have any questions about the survey, please reach out to Austin Lucas at austin.lucas@ohiospf.org. Thank you!
