Please respond to all the information requested on this form. Your data will be kept confidential and will be combined with other organizations' data before it is reported. Please complete one form for each program your organization operated in summer 2016.
This research is being conducted through a collaborative partnership among the Oakland Unified School District, Oakland Fund for Children and Youth, National Summer Learning Association, Partnership for Children and Youth, and a number of other community based organizations.
