Welcome to this Survey

ACON set up its Regional Outreach team in 2013 to work with communities and service providers to improve the health of people with HIV and lesbian, gay, bisexual, transgender and intersex (LGBTI) people and communities across southern and far western NSW. The outreach program is broadly known as Out&About

The activities of Out&About are focused on: HIV prevention and support, and LGBTI health. Priority LGBTI health issues have been identified as mental health, alcohol and other drug use, smoking, community safety and social inclusion, domestic and family violence, and ageing.
Out&About does this through health promotion (workshops, events, campaigns, social media), partnering with local community groups, peer outreach visits to sexual health clinics, at events and online, and working with local service providers. 

About the survey

This survey of service providers is part of the evaluation of Out&About, the ACON Regional Outreach Project. 

The survey closes on Monday 19th January 2016 at 6pm.  Please complete the survey before then!

It should take you 5-10 minutes to complete.  You will need to complete the survey in one sitting, as the survey will not save your answers.

The aim of the evaluation is to gather information about what is working well with Out&About and how it can be improved in the future.  This evaluation is to inform ACON's future planning for regional service delivery.  It is not tied to any funding agreements, or future funded work.

All information gathered through the surveys will be confidential.  ACON will be provided with a summary of key themes but will not be provided with individual survey data.
