This is an anonymous survey.

Please be as honest as possible, and use respectful language in your comments, if you choose to include them. Psychological safety is the foremost predictor for high-performance. Please focus your responses of the team of which you are a member. If you work in multiple teams, please choose your "main" team.

This survey is based on the extensive work of Professor Amy Edmondson, author of "The Fearless Organization."

You will be presented with a range of statements and will be asked to select a rating that best describes your response to the statement.

The survey is designed to be completed in less than 10 minutes.

In this SAMPLE MODE, your "score" will be emailed to you if you choose to provide your name and email address in the OPTIONAL section before you start the survey.

If you do not provide your name and email address, you will not receive a score but you will be able to assess the questions used in the survey.

The purpose of the sample is to enable you to see all the questions in the survey and how quickly the survey can be completed.

If you would like to use the survey for your team, please email Gary Ryan via to arrange for a specific survey to be created for your team or organisation. When the survey is completed by your team or organisation, you will receive a % score for your team and/or organisation's level of Psychological Safety. Your report will include potential actions to maintain, stop, or start in your team or organisation that will support Psychological Safety and aid high-performance.
