OTC Dog Adoption Application

Before applying, you should have the following already confirmed:
1. Real estate approval (if renting)
2. A rates notice as proof of home ownership (if owner)
3. Secure, suitable fencing and outdoor shelter
4. Ability to meet the pet’s dietary needs (breed size, age, medical requirements)
5. Capacity to provide appropriate enrichment, exercise, and training
This form is an expression of interest only and is not a contract. The application takes approximately 15 minutes to complete. Some applicants may take more or less time. This timeframe gives an indication of the level of detail required at the initial screening stage.
We do not approve meet-and-greets without a completed and approved application. Applications must be completed in full; incomplete applications will not be followed up. Required photos of yard and fencing must be provided within 48 hours or the application will not proceed. Photos may be sent via our Facebook page or emailed to offthechaink9rescue@outlook.com.
Due to the volume of behind-the-scenes work, we are not always able to contact applicants whose application is not a suitable match for the pet they have applied for, however we do endeavour to do so where possible. You are welcome to request an update or outcome at any time via email or our facebook page inbox and we will respond as soon as we can.
Transport Information
1. Transport is considered only after an adoption application has been submitted. Please apply before sourcing transport to avoid unnecessary arrangements and delays if you are not approved.
2. Submit an adoption application as outlined above.
3. Obtain transport quotes and select your preferred provider. We recommend Aeropets, Jetpets, Dogtainers, or Pet Waggin for road transport.
4. Advise us via email or Facebook message that you have applied and are beginning the transport sourcing process. This is a good time to advise any additional details, such as holding periods or limited departure days.
5. If your adoption application and transport plans are proceeding, we will coordinate departure, fit-to-fly/road requirements, food delivery, and any other necessary elements with you.
6. Adoption contracts, adoption fees, and photo ID must be submitted before the animal is released to transport.
7. Transport approval is subject to the individual animal’s suitability for travel and the standard of care provided by the selected transport company. Not all animals will be approved for transport. If an animal is not suitable to travel, this will be clearly stated on their adoption profile.
Once you submit your application an OTC member will endeavour to respond within 48hrs - however if you do not hear back from us please feel free to contact us for an update. Contact is generally via email or text. 15.How will you keep your dog secure and safe when you are not supervising?(Required.)

17.How many hours a day will your pet be unattended? We realise of course that a lot of people work and have commitments. Most of our pets are used to being alone part time. We just need to ensure the pet will cope with the level of human contact it will have.(Required.)

20.Have you made a plan for how you will train your pet to be alone safely and happily, when you return to work? (if applicable)
21.How much daily exercise and mental stimulation will you be able to give your dog and what activities will this include?(Required.)
22.Do you plan on letting your dog off-lead in unfenced dog parks / play areas / when walking? (While some of our dogs have excellent recall already, some won't be ready for unfenced play time just yet)(Required.)
23.Do you plan on allowing your dog in the house during the day? Where will it sleep at night? (eg inside on dog bed, inside in laundry, outside kennel, outside under patio - some of our dogs don't cope as outside-only dogs, whereas some are happy with anything. While we will not rehome our dogs to homes where some dogs are inside and some are out, as long as all dogs are provided the same sleeping arrangements there should be no issue. Please be honest in your answer)(Required.)

28.Have you owned any other animals, that may no longer be with you? If yes please briefly explain what happened to them.
29.Please tell us the name, street/suburb, and phone number of your current vet. Please write "none" if you don't currently have one.
30.If you have other animals in the house are you able to separate all animals at feeding time if required? Some dogs can be food possessive.(Required.)
31.Do you have experience with this breed of dog? Or at the very least have done research on their requirements?
32.Are you prepared to maintain the pet’s worming, vaccination and dental care/physical and mental health? Please remember that vaccinations are kept up to date while in OTC care but if you are adopting a young kitten/puppy they may not be fully vaccinated yet due to age. Any vaccinations due from the date of adoption are the ethical and financial responsibility of the adopter. (Required.)
33.Would it be acceptable for someone to visit your home prior to, during and after the animal has been placed? If you answer “No” please explain why.
