Intake Questionnaire

Thank you for your interest in Ontario Sex Therapy.

If you are interested in seeking services with our clinicians, please complete the following confidential questionnaire. Your responses will allow our intake team to assess whether the clinic is a fit for your needs.

This questionnaire is hosted by Survey Monkey, which is a secure platform. Your information will not be shared with any parties outside of this clinic.

Our intake team will follow-up via email once a questionnaire response is submitted. Our typical response is within two business days but may take up to a week. It is important you read all the information in that email and provide a timely response. Note that sometimes our emails end up in junk, spam, or Promotions folders, so please monitor these spots in your email.
