OPSI Best Practice Award 2017

The OPSI Best Practice Award was created to honor and recognize successful patient safety activities in Ohio health care institutions.

Criteria includes:

--Patient safety best practice project summary
--Performance improvement process
--data analytic approach (Measurable goal established,use of benchmarks, etc)
--Results and sustainability
--Lessons learned

The winner and finalists will be announced in March, and the award will be presented in June at OHA's Annual Meeting.

Please submit any relevant charts or graphs (no more than 2 figures) to Rosalie Weakland at rosalie.weakland@ohiohospitals.org

The deadline to submit is December 31, 2016
